Do you accept insurance?

We accept:

Self pay:
I provide a superbill and invoice to submit to your insurance company to be reimbursed.

How much is a visit?
Self pay rate is based on length of appointment and type of service provided. Please inquire with the office.

Your card on file is charged at the time of visit.

Missed appointments
Appointments must be cancelled or rescheduled within 24 hours of your appointment. Not arriving to appointments is a $50 fee and cancelling with less than 24 hours notice is a $25 fee.

Please request refill requests via your patient portal, log in above. I review refill requests, if you have missed an appointment refills may not be provided.
