Do you accept insurance?

Yes!

We accept:

- Optum network (Connecticare, United, Oxford, Harvard Pilgrim)

- Aetna

- Cigna

- Anthem/BCBS

- Medicaid (Husky)

Self pay:

I provide a superbill and invoice to submit to your insurance company to be reimbursed.

​

How much is a visit?

Self pay rate is based on length of appointment and type of service provided. Please inquire with the office.

Your card on file is charged at the time of visit. 

​

*Sliding scale based on income is available

Missed appointments

Appointments must be cancelled or rescheduled within 24 hours of your appointment. Not arriving to appointments is a $50 fee and cancelling with less than 24 hours notice is a $25 fee.

You will receive reminders via text and email prior to your appointment.

How can I get a refill?

Please request refill requests via your patient portal, log in above.  I review refill requests, if you have missed an appointment refills may not be provided.
